Liability Release:

In consideration of participating in Itty Bitty Bakers LLC’s class, camp or special event, I acknowledge and understand the dangers and risks inherent in such activities related to preparing food, consuming certain foods and working with tools and appliances. By signature of this Itty Bitty Bakers Waiver of Liability and Release (the “Release”), I hereby waive, release, indemnify, and discharge Itty Bitty Bakers, its instructors, officers, employees and volunteers against any and all legal claims, demands, action or causes of action for any and all costs, expenses or damages to personal property or personal injury, or death, which may result from my participation or child’s participation in these activities. 

In the event of any lawsuit, the parties agree to submit any dispute for resolution by arbitration through the Alabama Bar Dispute Resolution Services of the Alabama Bar Association and any award rendered, if any, shall be final and conclusive to the parties.

 

I assume full responsibility for any injuries or damages resulting from my participation or child’s participation in this program including responsibility for using reasonable judgment in all phases of participation of the program. I acknowledge that my participation or child’s participation in any and all Itty Bitty Bakers activities is solely at my own risk, and that I assume full responsibility for any resulting injuries and damages. I understand that it is my sole responsibility to notify the appropriate person at Itty BItty Bakers of emergency medical information and have informed the Itty Bitty Bakers instructor of any food allergies/or dietary restrictions for my child. I also understand that this Release binds my heirs, executors, administrators, and assigns as well as myself.

Media Release:

I grant permission to Itty Bitty Bakers, LLC, its agents, and its employees the irrevocable and unrestricted right to produce photographs and video taken of my child during class or camp for any lawful purpose including publication, promotion or advertising in any manner or in any medium by Itty Bitty Bakers. I agree to their right to use or reproduce such photographs and videos in any media including print materials, social media, or the website. If I do not wish to have photographs or videos of my child used in media publications, I will leave the box below unchecked to signify my lack of release.

Health and Safety Release:

• I understand that Itty Bitty Bakers classes and camps cannot be 100% free of food allergens and it is at my own discretion whether or not my child attend. I will not hold Itty Bitty Bakers liable should my child have any allergic reaction during a class or camp. I agree to notify the Itty Bitty Bakers Food Allergy Support Team via the Food Allergy Notification Form below of any food allergies. I will also notify my child's specific teacher on the day of class or camp each time that he/she attends.

• I agree to keep my child home if he/she or anyone in my family is coughing, has a temperature of or over 100 degrees Fahrenheit, Covid-19 symptoms or has been exposed to anyone with Covid-19 symptoms.

• My child will have regular opportunities to wash hands during the class, camp or special event. My child can also request to wash hands when needed/wanted. My child will use the restroom and wash his/her hands thoroughly before leaving home. I will have my child wash hands thoroughly upon arriving back home.

• I am aware and agree that close watching and occasional hands-on instruction is an essential part of teaching my child in addition to keeping him/her safe and to prevent injury. I will allow this instruction when necessary. 

• In case of an emergency, I grant any staff of Itty Bitty Bakers permission to seek medical care for my child.
